Intro

SWACH (Southwest Washington Accountable Community of Health) is an organization serving Clark, Klickitat, and Skamania counties, including native communities and tribal lands. We promote health equity by collaborating with partners, providing healthcare access and social support, offering substance use services (including free Narcan), funding community events and organizations, and supporting whole-person health programs. Our annual Community Needs Survey, open to residents 18 and older, takes about 8 minutes to complete and is anonymous unless you provide contact information. This survey helps guide our programs, funding, and policy priorities, with results shared publicly. Your input is crucial in shaping our community health initiatives, which are funded through various sources including Medicaid innovations and state/federal funds.

If you have any questions about this survey, please reach out to heather.sheppard@southwestach.org
