The Evaluation Appeal online form is required in order to advance an appeal, and is intended to assist with ensuring that adequate information is provided; such that the Appeal Committee can make an appropriate determination.

STEP 2 - There is a $300 Appeal fee (non-refundable/at the discretion of the Eval Committee). This fee must be e-transferred to: treasurer@southwesthockey.ca in order to move this appeal forward.
