Kit for New Parents: Distribution Order Form

Note: Please use this form when requesting multiple First 5 Contra Costa Parent Kits. Our Parent Kits are custom to Contra Costa County and are bilingual, with materials in both English and Spanish.

Pickup options: Kits are available in bulk for pickup from First 5 Contra Costa at 2300 Clayton Road, Suite 300, Concord, CA 94520. Pickup must be scheduled in advance. Please provide your preferred pickup date and time, and we will do our best to accommodate your request. First 5 staff will follow up by email to confirm the pickup date and time and coordinate details.
