All committees and task forces should use this form to report the results of their long and short-term activities and projects, including those prescribed by their committee’s charge.

You can submit all activities/projects on a single form. There is space on this form for 5 separate activities/projects. If you have more than 5, please fill out a separate form for those additional activities/projects.
