AMS 2023 Tax Organizer Questionnaire

This survey is a short series of "Yes/No" questions which will allow AMS accounting staff to prepare for any changes that may impact your tax return preparation. We only need your full name/and or company name in this form.
Do NOT enter any addresses, social security numbers, EIN, TIN, or account numbers, income information, or any other personal information into this form, except for your name and/or company name.
PERSONAL INFORMATION

3.Did you live separately from your spouse during the last six months of the year?
4.Do you have a separate decree, instrument, or agreement and are not living in the same household by the end of the year?
