Thank you for taking the DHS Exit Survey. The purpose of this survey is to understand the reasons you have chosen to leave employment at your current organization within DHS. Your responses will help DHS identify areas for improvement. All responses will be CONFIDENTIAL. Responses are NOT linked to personal characteristics/identifiers (such as name, email, or other personal attributes) and will be reported in summarized/aggregate form only.  

The survey will take approximately 10 minutes to complete. Your participation is voluntary. Please note that any questions marked with an asterisk (*) are required in order to continue with the survey. All other questions can be skipped if you do not wish to answer.
