Introduction
The information you provide in this survey will be collected and used by MOGAD Eugene Devic European Network (MEDEN - https://www.meden-eu.org/), with support from The Sumaira Foundation (TSF), using Survey Monkey, a secure web-based platform. Your answers will not include your name or any personal details and will be grouped with responses from other participants. Your answers will be seen by physicians responsible for the study, but no identifiable information will be shared. The combined results may be used in publications aimed at both patients and researchers.

The data you provide will only be used to manage your participation and help analyse the overall survey results. We will not collect any information beyond what you choose to provide in this questionnaire. The data will be accessible only to MEDEN members to understand the research priorities of people living with NMOSD and MOGAD.
