Thank you for taking part in our evaluation, to share your insights and story about the GHCF funding you received and how you were able to use it.

DG Cities is undertaking an independent evaluation and what you share will remain confidential (unless you give permission to share specific information, such as for a case study). This follows our data & privacy policy, which you can find here.


We encourage you to share transparently with us in a way that can support any improvements to the fund, shed light on health inequalities or help others in the future. Your insights are really valuable and appreciated.

Here are some useful things to know! -
  • There are multiple choice questions, as well as open questions where you can provide longer comments and insights.
  • Some questions are optional for you - the question text is followed by '(Optional Question)'
  • You may start the questions then pause and return to complete the survey (from the same device) if you need.

Alongside the questionnaire, as per the evaluation guide, you are very welcome to share further data about the impact of your grant if you would like with the evaluation team, at info@dgcities.com.
