6.I have voluntarily elected to allow my child(ren) to participate in activities at the DLA Susquehanna’s Annual Egg Hunt March 14, 2026 (the “Event”) and I fully understand that there are health and safety risks associated with the activities. In consideration for receiving permission to participate in the various children activities mentioned below, I, the undersigned, for myself, my heirs, personal representatives or assigns, do hereby release, waive, discharge, and covenant not to sue the US Government, DLA Distribution Susquehanna, Pennsylvania, Department of Defense, Defense Logistics Agency, DLA Distribution, and all other related government agencies and/or their employees, agents, and representatives associated with DLA Distribution’s Organization and the Event (the “Releasees”) for any and all claims of liability for personal injury, accident or illness (including death) and property loss arising, either in law or in equity, which arise or may hereafter arise from my child’s participation in the Event. I understand and acknowledge that this agreement discharges any and all, from liability or claim that my child may have with respect to bodily injury, personal injury, illness, death, or property damage that may result from my child’s participation in the Event, regardless of whether such loss is caused by the negligence of the Releasees.

In consideration of my child’s participation in the Event, upon the terms hereafter stated, I hereby grant Defense Distribution Center Susquehanna's Family and Morale, Welfare, Recreation and/or its agents and their respective licensees, successors and assigns, the absolute right and permission to copyright and use, re-use and publish, and republish still photography or video footage of my child or in which may be included, in whole or in part, without restriction as to changes or alterations, from time to time, in conjunction with my child’s name, or reproductions thereof otherwise made through any media now known or to be devised for documentary, promotional, artistic, marketing, trade or any purpose whatsoever.
