The School Discipline Survey: Educators

Introduction
We are interested in the types of discipline that students are receiving in their schools. This survey will ask about your school's discipline policies and your own experiences with your school's discipline policies. By doing this study we hope to learn about school discipline practices in Texas. Please read the information below before participating in the study. If you click “yes”, this form will be used to record your permission.

What will you do?
If you participate in this study, you will be asked to complete one 20-minute survey about your experiences with school discipline in Texas schools. We are asking youth, parents, and educators all across Texas about their experiences with discipline in school. 

If you choose to, you can also give us your contact information so that we may do a follow-up interview with you to learn more about your experiences. Giving us your contact information is optional.

What are the possible risks and benefits involved in this study?
There are no foreseeable risks to participating in this study, nor will you receive any direct benefits from your participation. Data obtained from the survey may help Texas Appleseed, Easterseals, and Disability Rights Texas develop more effective discipline policies in K-12 schools across Texas.

What if you do not want to participate?
Your participation in this study is entirely voluntary. You can decline to participate or withdraw from the survey at any time. Even if you initially agree to be in the study, you can change your mind later without penalty. If you decide to withdraw, your data will not be included in the study.
How will we use your response? 
Your responses are confidential and private - meaning that we will not release any information about you. Once the study is completed, we will write an evaluation about how all participants responded. The evaluation will be used in a report on the state of discipline in Texas.

How will my privacy and confidentiality be protected if I participate in this study?
Our staff will protect your privacy and confidentiality of your data by assigning each participant an anonymous study ID number. All information collected from you will be stored under anonymous study ID numbers in secure files. No information about any individual will ever be disclosed to the public. Once the study is completed, we will write an evaluation looking at how all the participants answered.
