Who can complete this survey?
This survey is for parents or primary caregivers of children in grades 3–6. We only need one person to complete the survey for each child.
Why am I being asked to complete this survey?
Your school is participating in the Kids Active Travel Program, which is funded by Sport and Recreation Victoria who want to understand if the Program is working.
What is the survey about?
This survey will ask about your child's travel habits and your views on children walking or riding to school. It takes less than 10 minutes to complete. If you have more than one child in grades 3–6 at the school, please complete a separate survey for each child.
The survey is being run by Sport and Recreation Victoria, part of the Department of Jobs, Skills, Industry and Regions (the Department). The survey has received Research in Schools and Early Childhood Settings (RISEC) approval, which is managed by the Department of Education and Training.
Is participation voluntary?
You don't have to complete the survey. If you don't complete the survey will not affect your relationship with the school. If you complete the survey and change your mind and want to withdraw, email getactive@sport.vic.gov.au within one month of completing this survey.
How will my privacy and confidentiality be protected?
Data collected through this survey will be used to understand the impact of the Kids Active Travel Program and to guide potential future investments in the Program.
The survey will not collect information that will be used to identify you or your child. The survey is hosted by Survey Monkey. Data will be stored securely by the Department for a minimum of 5 years to evaluate the Program, then it will be destroyed.
