The Durham District School Board, in partnership with community agencies, offers extended-day programs in most DDSB schools.

This survey is designed for families who are interested in accessing extended-day programs for children between the ages of 4 and 12 years of age. The purpose of this survey is to better understand the Before/After School Program needs for children who will be registered for the upcoming school year.
Please note that this is not a waitlist or registration form. Completing this survey does not guarantee that an extended day program will be offered at your child’s school, nor does it determine admission priority, should a program be offered. The information collected in this survey will help the DDSB identify where new or expanded programs may be needed for schools who do not currently have a variety of extended day programs available. If there is sufficient interest for a particular program based on the outcomes of the survey, families/caregivers will be notified through a separate process.
