Instructions

Please use the following survey to submit data related to your:

  • Third year’s application of approved treatment (if applicable).
  • Harvest of fruit following the third year’s application of approved treatment.
  • Submittal of Attachment 3 - Claim Form

Each contract should submit a separate survey.

CRAFT contract numbers can be found in the top right corner of your participation agreement. If you do not know your contract number, please contact Tina Buice at tina@craftfdn.org, Carisa Keller at carisa@craftfdn.org, or Steven Hall at steven@craftfdn.org.

As you move through the survey, please do not use your browser's back arrow to return to previous pages; please use the "Previous" and "Next" buttons found at the bottom of each survey page.
