Instructions

Please use the following survey to submit data related to your:
  • First injection or treatment and
  • First post-injection or treatment harvest season production data
  • Second injection or treatment and
  • Data for the season, two years post-second injection

Each contract should submit a separate survey.

CRAFT contract numbers can be found in the top right corner of your participation agreement. If you do not know your contract number, please contact Tina Buice at tina@craftfdn.org.

As you move through the survey, please do not use your browser's back arrow to return to previous pages, please use the "Previous" and "Next" buttons found at the bottom of each survey page.
