Employee Resource Connect is a program to help identify employee needs and build a healthier community. Please answer a few quick questions on food, housing, transportation, behavioral health, and childcare. This survey qualifies for points for the Choices for Health Wellness Program activity. All information will be confidential and only shared with Family Service Agency (FSA), a community organization who can help connect you with local resources. If the survey indicates resources might be helpful to you, you’ll be offered options for accessing individual support through FSA.
