This survey is being carried out by NAVCA* (the National Association for Voluntary and Community Action) on behalf of the Health and Care Voluntary Sector Strategic Partners**.

We would like to find out about the extent to which groups of people with different needs are represented on local health structures. This information will form part of a report regarding representation of voluntary and community organisations and their beneficiaries on these health structures. This will be shared with the Department of Health, NHS England and the NHS Commissioning Board, as well as other Strategic Partners and the wider voluntary and community sector.

All information will be kept confidential, and we will not share your personal details outside of NAVCA without your consent.

**The Voluntary Sector Strategic partnership is a programme that aims to improve transparent relationships between the Department of Health, NHS Commissioning Board and Public Health England and the voluntary and community sectors and increase capability and knowledge within the voluntary and community sectors.
