The purpose of this survey is to collect preliminary information from individuals interested in becoming an EQT Community Ambassador for efforts at the local level ( south side of Madison) by amplifying the voices of those impacted by health inequities. EQT is seeking individuals who can engage with people at both a community and grassroots level, as well as those who have existing relationships and connections in one of Madison's many diverse communities.

Please note this is not a job, but a community role in which we will provide a fair financial incentive for your assistance.
