End of Life Bereavement Care Survey |
If you have received a bereavement booklet, it is likely that a relative of friend has recently died whilst in the care of South Eastern Health & Social Care Trust. We would like to offer our condolences at this difficult time to you and your family.
It is important for us to be aware of how the care was delivered to your relative/friend in and around the time of death and immediately afterwards. With this in mind we are inviting you to complete this short questionnaire which provides you with an opportunity to share your experiences.
You are under no obligation to complete the questionnaire and all replies are anonymous. The information that we gather will be used to help monitor and review the care that we deliver at this important time.
