2026 ACHSM EOI to create a Health Coaching Panel

Please note that completing this EOI does not guarantee that you will be contacted or engaged to deliver coaching services on behalf of ACHSM - we are creating a Coaching Panel to provide visibility of our members who are professional coaches.

6.This EOI is only open to ACHSM members who are practicing coaches and current Associate Fellows (AFCHSM) or Fellows (FACHSM) of the College (as a means of ensuring your experience as a Health Manager and Leader). Please clarify which of this is relevant for your membership.(Required.)
7.Please indicate which Coaching Streams you would be interested in working within.(Required.)
8.If you have nominated Leadership Coaching, please list the development tools that you are currently accredited to administer and debrief.
9.Please provide a brief outline of your coaching qualifications, experience and expertise and interest areas.(Required.)
10.Please indicate the locations in which you would be able to deliver virtual coaching sessions.(Required.)
11.Please indicate the locations in which you would be able to deliver face to face coaching sessions.
12.Please provide the details of two professional referees who can comment on your coaching experience (noting they would only be contacted if you were going to be engaged by ACHSM to deliver coaching services on behalf of the College). Please provide their names, email contact, phone number and an explanation of how you have worked with them previously in a coaching context).
13.In completing this EOI, it is assumed that if you were engaged to deliver coaching services on behalf of ACHSM, that you would be willing to comply with the confidentiality and ethical requirements that the College would expect to be maintained.(Required.)
