Thank you for taking the time to complete this survey. Your feedback allows us to adjust Safe Routes to School (SRTS) education and encouragement program features to best meet the needs of each individual school. Your feedback will also be shared with decision makers who can address infrastructure issues regarding safe walking and biking.
Please use this form to provide information about one of your children or more than one of your children, as long as they attend the same school.
If your children attend different schools, please fill out one survey for each child.
