1. Purpose

The objective of this survey is to understand the use of health information technology (HIT) systems, specifically Electronic Medical Records (EMR)*, in nursing homes in New York State and to identify barriers to further implementation. The Department of Health (Department) is especially interested in the capacity for the State’s nursing homes to collect, share, and analyze clinical information electronically.
 
The information collected from this survey will be used to inform policy and decision-making regarding HIT in New York’s long term care settings.
 
We request that the Administrator, or an equivalent title with decision making authority over and familiarity with IT matters, complete this survey. Please submit only one survey for each facility.
