Virtual Services Survey

Empower Cherokee would like to provide virtual services to all person interested in participating as an alternative to in person services. Please take a moment to fill out this survey so that we can appropriately plan for this new service.

People who use our traditional services would be eligible to participate in virtual services only, both virtual and in person, or in person only up to the amount of services allocated per the persons support plan.

This is an exciting alternative to in person services during this uncertain time.
