Thank you for taking the time to complete Dudley Council’s survey about dummy use. The information you share will help us understand current experiences and support the design of a new Public Health scheme that meets the needs of local families and communities

Do you have a child under 5 years old who does or did use a dummy? If so please complete our survey.

If you have more than one child under 5 years old who does or did use a dummy, first please tell us about your youngest child who does / did. When those questions are completed you will have the option to tell us about another child under 5 years old who does or did use a dummy.

The survey will take about 5 to 7 minutes to complete.

The prize draw opens at 9 am on 14 August 2026 and closes at 5 pm on 18 September 2026. Late entries will not be accepted.

Your responses to this survey will be confidential and anonymous and you will not be asked for any personal information (such as date of birth, address, ethnicity). If you want to enter the competition you will be asked to give us an email address where you can be contacted. This email will be used solely for the purposes of getting your £20 shopping voucher to you if you are a competition winner, and will be deleted once the winner is chosen and contacted.
