Survey to Develop Discharge Criteria for Endocrine Conditions
The CSEM QI Committee is looking for your help on a project aimed at improving ambulatory care transitions from the endocrine to primary care setting. We are looking to develop a set of discharge criteria for some of the common endocrine conditions that we see in practice. Our hope is that this initiative will improve patient experience and help to standardize workflows, both for Endocrinologists and our colleagues in Primary Care.
Your responses will support the QI Committee in two ways:
1) Assist with identifying endocrine conditions that could support discharge criteria
2) Participate as a Delphi panel member
Volunteering as a Delphi panel would involve completing two surveys; one at the beginning and one at the end. It would also involve attending a synchronous zoom meeting. Thank you again for your support of this important initiative.
