Introduction

Silicon Valley Community Foundation's Center for Early Learning has partnered with American Academy of Pediatrics (AAP) Chapters throughout California to learn about your views and practices regarding early childhood development services.  This statewide survey will capture information on the attitudes and practices of California health care providers concerning early developmental screening. We thank you in advance for taking approximately 15-20 minutes of your time to complete the following survey. Your feedback will provide us with critical data to plan and develop comprehensive policies and services that support children and families in California to grow and thrive.

The survey includes questions about the developmental screening attitudes and practices of your office.  For the purpose of this survey, developmental screening is defined as a first line check of a child’s development and behavior to identify possible developmental delays and/or other special needs. The information you provide will be used to inform planning and development efforts to help support you in the important work you do to serve the needs of California’s children and families. 

We ask that you complete the survey by November 11, 2016.

As a thank you for completing the survey, you can enter a drawing to receive one of a range of prizes including $15 or $20 Starbucks gift cards or one of the 30 FREE Nexus 7 Tablets! Up to 50 respondents can win a prize!

Only one survey should be submitted on behalf of your practice. Any staff member who is familiar with the developmental screening practices of the office may fill out the survey, including physicians, nurse practitioners, nurses, and medical support staff.  Please ensure that the person who completes the survey is sufficiently informed about office attitudes and practices to respond to every question.  All individual results will remain confidential; only aggregate results will be reported.
