KANE COUNTY SCREENING & OUTREACH COMMUNITY ASSESSMENT |
You are receiving this survey as a joint partnership between Kane County AOK Network Developmental Screening Workgroup, SPARK Aurora Early Childhood Collaboration, Elgin Partnership for Early Learning and DayOneNetwork. To support and inform our work, there is a need to gather more information on the developmental screenings occurring in the Kane County area. This survey will take approximately 15 minutes to complete and will provide the community with a programmatic and environmental scan of services being offered and available in the Kane County area. If you are not the appropriate contact to complete this survey, please forward to the appropriate personnel within your agency. Aggregate results from the survey will be shared with all respondents by contacting any of the below organizations. Personal information will not be shared.
Programs and Agencies receiving this survey include: Health, Education, Prevention Services, Child Care, WIC, etc. One of the goals of the partnership is to develop a community wide systemic approach designed to increase awareness, access, and utilization of developmental screening for children 0-3 and 3-5 years of age. Your response will inform our planning, work, and innovations.
A screening tool is designed only to identify children who may need additional assessment in order to determine their progress on age-appropriate norms. Screening tools are intended to be brief in administration, used with a large population of children, and contain items that examine broad developmental milestones. Screening tools are not designed to assess a child’s specific strengths and weaknesses in various domains, to give guidance for planning intervention, or to plan program improvements. A screening tool is not to be confused with an assessment instrument.
