CONSENT TO PARTICIPATE IN THE BRAND AUDIT OF THE DETROIT RED WINGS
I understand that I have been asked to participate in a research project being conducted by a student group of the MBA program of Concordia University (contact info), under the supervision of Professor Zeynep Arsel of the Department of Marketing of Concordia University (email@example.com Phone:(514) 848-2424 Ext: 2956 ).
I have been informed that the purpose of the research is as follows, an analysis of the Detroit Red Wings brand with consideration given to the recent NHL lockout.
I understand that I will be asked to answer questions in an online survey format that will require approximately 10 minutes of my time and all safeguards are being taken to protect the confidentiality of my participation.

• I understand that I am free to withdraw my consent and discontinue my participation at anytime without negative consequences.
