A community engagement project has been developed to help make death, dying and loss, less of a taboo. The purpose of this survey is to record current attitudes towards death and to then re-evaluate over the coming months. Engagement activities will also be occurring over the next year, in the hope it will encourage people to be more open about discussing death, and for death to be accepted as a natural part of life.

Data collated from the surveys will help formulate a project report. Personal information will be stored in accordance with the Data Protection Act. This page will be kept separate from the answer sheet.
