INFORMED CONSENT

PURPOSE
This is a research study investigating the opportunities and barriers to data conceptualization, collection, and utilization by non-profit organizations. The purpose of this study is to better understand what kinds of data non-profits are currently collecting and for what purposes, what types of data would be most desired or useful for your organization to collect, and what you wish funders understood about program monitoring and evaluation. Approximately 30 non-profit leaders, staff, and stakeholders are expected to take part in this study.

PROCEDURES
If you agree to take part in this study, your involvement will last for approximately 30 minutes. You will be asked to complete a series of questions including demographic information and your understanding of data collection and analysis. You will also respond to questions that assess what forms of data you use in your decision-making processes, and how you connect collected data to program efficacy. The measurement will take place via online survey software, SurveyMonkey.

You will receive a unique participant number in order to participate in this study. This participant number will be connected to your personal information such as your name, organization, and contact information. However, your responses will only be viewed by the researchers and will be reported in group-form only.

RISKS & BENEFITS
There are minimal foreseeable risks to participating in this study. Your responses on the survey will be confidential and protected to the full extent of the law. Data from this study will be analyzed and reported in aggregate-form only. Your name or how your responses tie you to your specific organization will never be included in any reported results.

In this study, you will be asked about your personal characteristics and organization's characteristics. You are free to skip any item that makes you feel uncomfortable.

You may not benefit personally from being in this study. However, we hope that other non-profit organizations as well as funding agencies will benefit from the findings gained in this study.

VOLUNTARY PARTICIPATION
Taking part in this research study is completely voluntary. You may choose not to take part at all. If you decide to be in this study, you may stop participating at any time. If you decide not to be in this study, or if you stop participating at any time, you won’t be penalized or lose any benefits for which you otherwise qualify.

CONFIDENTIALITY
Records of participation in this research study will be kept confidential to the extent required by law. All data and consent forms will be stored in a secure area. In the event of any report or publication about this study, results will be reported in a summarized manner so that you cannot be identified.
