The Montgomery County Board of Developmental Disabilities Services (MCBDDS) would like your opinion regarding the quality and type of services it has provided during the past 12 months to you or your family member.  We would like individuals receiving services to complete this survey as independently as possible, with family members/guardians completing a separate survey.  You can take the survey online at https://www.surveymonkey.com/r/DZ883LF or download a copy to print at http://www.mcbdds.org/survey.  

This survey only has 16 questions.  Please answer all that apply to you, and return it no later than Friday, July 29, 2021
 
If you wish to print and return the survey via U.S. Mail, please send it to: MCBDDS Communications Dept., 25 Thorpe Drive, Dayton, OH 45420.  If you receive this survey from a provider, you can return it to your provider, and they will forward it to the Board.
