Benefits Include: 
  • Receive seed funding to implement “Learn the Signs. Act Early.” in at least 3 WIC clinics.  The model used could include in-person, virtual, or a hybrid model

Participation Requirements: 
  • Develop a State/Agency team (see info below)
  • Short (30 min) monthly meeting between state point of contact and ASPHN project manager
  • Submit brief progress reports (Jan, April, July)
  • Participate in implementing LTSAE into at least 3 WIC clinics
  • Attend a monthly training webinar or call provided by ASPHN (will be recorded).

Selection Criteria:
  • We will be selecting states/agencies based on experience with LTSAE, willingness to participate in implementing LTSAE in multiple clinics, plans to continue expanding the program, and previous participation in the WIC Developmental Monitoring Project.

* 3. Tell us about your WIC program’s experience with “Learn the Signs. Act Early.” (LTSAE) or a similar early child development program. If you have been a part of the WIC Developmental Monitoring Project as a Technical Assistance state, please describe your participation in it. (3-5 sentences)

Should your state/agency not be selected to receive funding, or your state/agency cannot commit to the intensive program, would your state/agency be interested in joining the learning collaborative as a Technical Assistance (TA) state, where you can benefit from technical assistance and learning from other states, but not have the requirements of an intensive state? * 5. ASPHN is looking for states who are willing to implement LTSAE into at least 3 WIC clinics with the potential of implementing into more clinics in the future. Is your state/agency willing to consider implementing in additional clinics in future years should funding/time become available? * 8. We will be working with states from October 2022- July 31, 2023. Each state/agency will receive support in the form of funding and technical assistance to be used throughout this time. States/agencies will not receive actual money, however ASPHN will manage the funds and purchase the materials on their behalf. This support will include program materials and costs to implement LTSAE. We will select states that are committed to continuing implementation beyond the funding period.

 

We would like to provide funding to implement LTSAE into a few clinics or expand the program to additional clinics to as many states/agencies as possible. ASPHN has a total of $30K to support all agencies in this project. Please provide an estimate of how much funding your state would need to implement the program in a few clinics or to expand the program in your state/agency.

 

Potential costs include:
  • Printing WIC Customized Checklists
  • Printing Environmental Graphics for each clinic
  • Printing LTSAE posters, milestone tracker app flyers, brochures, etc.
  • Printing Deskside Reference Guides for WIC staff
  • Supplies (folders, file boxes, etc.)
  • Costs to train WIC staff (in person or virtually)
  • Creating state-specific online milestone checklists (See NV’s online checklists for an example)

* 9. LTSAE will be integrated into at least 3 clinics in your state/agency. Please provide the clinic and city name, monthly caseload, and primary language spoken in each clinic. (This should include at least 3 clinics)

* 10. If your state/agency is accepted into the WIC Developmental Monitoring Program, you understand that the state/agency team will need to:
  1. Have a state/agency team assembled prior to submitting the application. More team members can be added throughout the project.
  2. Designate a team member as the State/Agency Team Point of Contact Person. The responsibilities of the State/Agency Team Point of Contact include:
    • Meeting with the ASPHN project manager approximately once a month until the project period has ended.
    • Submit brief periodic progress reports (Jan, Apr) and a final report (Jul).
  3. Attend a monthly training webinar or call provided by ASPHN (will be recorded).
  4. Provide training to the 3 clinics on how to implement LTSAE.
  5. Participate in implementing LTSAE in at least 3 clinics.
  6. Track enrollment and compile information about implementation from the clinics to share with ASPHN.
  7. Be responsible for producing a plan to expand the program to additional clinics in future years should the funding/time become available to do so.

I understand that each clinic will be required to:
  1. Participate in a training on how to implement LTSAE into their clinic.
  2. Implement LTSAE in their clinic.
  3. Track implementation by recording the number of checklists distributed, completed, and referrals made as a result, and provide feedback about the experience by completing a brief survey or interview.
  4. Be responsible for adapting the model to fit into clinic flow with technical assistance provided by ASPHN consultants and CDC.
