Alberta's Minister of Health has requested that all health care colleges collect data related to COVID-19 from their registrants. By collecting essential information from health care practitioners the Ministry can understand workforce capacity and better facilitate their emergency responses.

The Alberta College of Opticians of Alberta and the Minister of Health need to collect as much data as possible to effectively manage the ongoing pandemic. Prompt and accurate registrant responses are extremely valuable.

We ask that you submit a response to this survey as soon as possible based on your current situation and be prepared to submit additional information as it is requested.

Thank-you.
