In our quest to continue to improve the wellness of all students within Lompoc Unified School District (LUSD), we are asking you to complete this short survey, and to provide us with your input on our Wellness Policy, and your child(ren)'s participation in school breakfast, lunch and physical education activities. 

If you have multiple children who attend different schools within our district, please complete one survey for each school.
