Local Public Health Hero Award Nomination

Purpose

In celebration of National Public Health Week, the Minneapolis Health Department (MHD) would like to recognize the accomplishments of community partners who help raise awareness of the critical roles that public health and prevention play in making Minneapolis a healthier place to live, learn, work, and play.

Process

  • Please submit your nomination(s) for Local Public Health Hero awards by filling out this nomination form.
  • Submissions are due Monday, March 19 at 5PM.
  • Winners will be notified no later than Tuesday, March 27.

Outcomes

  • Local Hero Award winners will receive their awards at the Local Public Health Heroes Award Ceremony to be held on Tuesday, April 10, 2018 at 11:30 a.m. in the City Hall Rotunda.
  • One or more awards may be awarded in each category of the Minneapolis Health Department goals.
  • Local Public Health Heroes will be chosen based on their work and dedication to one of the Health Department’s Goals.

Eligibility and Guidelines

  • Nominations can be for individuals, teams, or organizations in recognition for work on policy change,programs, services, or other efforts.
  • Nominations will be evaluated by the following criteria: 1) length of time since last Public Health Hero Award, if applicable; 2) strength of relationship with the Health Department; 3) exemplification of goal area; 4) work towards health equity; 5) public health impact; and 6) innovative practice(s).
