UserWorks, Inc., a user experience design and usability research firm, is currently recruiting taxpayers for a study to evaluate the usability of a website related to federal taxes.
The questionnaire on the following pages is being used to qualify people to participate in this usability evaluation. The study is being conducted on March 22 through March 26, 2021, and would involve you connecting with a moderator by phone and the Internet. It can be done from your home or office and takes about 75 minutes. This includes set-up as well as the actual test session. You would be paid $100 upon completion -- your choice of a mailed check or an emailed Amazon gift certificate.

Please make sure when completing this questionnaire that you enter your email address and telephone number correctly so that we can reach you.
An important note: Your answers to these qualification questions and all the information you provide during the subsequent phone call and eventual product evaluation session will be treated as strictly confidential. Your name, contact information, and responses that would identify you personally will not be shared with our client or anyone else. Data will be reported only in the aggregate or after personally identifiable information has been removed. You will not be asked to purchase anything.
The authority requesting the information is 5 USC 301. The primary purpose of asking for the information is to determine steps the IRS can take to improve our services to you. The information may be disclosed as authorized by the routine uses published for the following Privacy Act System of Records: (1) IRS 00.001 Correspondence Files and Correspondence Control Files and (2) IRS 10.004 Stakeholder Relationship Management and Subject Files. Providing the information is voluntary. Not answering some or all of the questions will not affect you.
