Award Nomination General Information
Thank you for making a nomination for the 2020 Des Moines Civil and Human Rights Symposium Awards. All are welcome to make nominations for the awards. However, eligible applicants should have an impact on the Greater Des Moines area.
The nomination process contains 6-8 questions, including one (1) free-form section for you to support your nomination. You may type your answer in another program or application and then copy-and-paste the information into this nomination form if you wish.

Each award has different terms of eligibility. Nominees who do not meet the specified terms of award will be disqualified.
Deadline: All nominations are due by 11:59 p.m. on December 22, 2019. Nominations that do not meet this deadline will not be given consideration.
