The Texas Department of State Health Services (DSHS) is conducting a survey of external partners and stakeholder organizations to determine how well DSHS is doing as an organization to accomplish its mission, and as part of this, how well DSHS is engaging stakeholders.

The DSHS mission is to improve the health, safety, and well-being of Texans through good stewardship of public resources, and a focus on core public health functions. DSHS Strategic Plan (2017-2021) goals are to:
1.   Improve health through prevention and population-health strategies;
2.   Enhance public health responses to disasters and disease outbreaks;
3.   Reduce health problems through public health consumer protection; and
4.   Expand the effective use of health information.

Over the past two years, some DSHS programs and functions have been transferred to the Health and Human Services Commission and other state agencies, streamlining the agency’s focus on public and population health.  As we align our responsibilities for this renewed emphasis, we value your input and thank you for sharing your thoughts about our work.  We plan to use your feedback to assess our strengths and weaknesses, identify areas for improvement, and inform future initiatives.  

This survey should take between 5-10 minutes to complete. The last question is open ended. Please use that space to provide any additional information that is not fully captured in the other questions.
