Jessamine County Health Department Services/Program Follow-up survey

The Jessamine County Health Department is seeking feed back from our community to evaluate overall community knowledge and awareness of the Jessamine County Health Department's programs and services.  This survey is a follow-up survey that was disseminated in April/May 2014.  Please take a few minutes to answer this short 8 question survey.  If you did not fill out the previous survey, we still want your feedback.  Thank you!
