This award recognizes members, nonmembers, and entities for long-time dedication to advancing, contributing, and supporting to the MAFP and the specialty of family medicine. It is intended to recognize long-time dedication and efforts in advancing, contributing or supporting the MAFP and the specialty of family medicine -- rather than through a single, significant contribution and effective leadership in furthering the development of family medicine. This award is not limited to members only.

The deadline for nominations is April 30. To ensure your nominee is eligible for this award, please review the judging criteria, eligibility requirements, and limitations below BEFORE completing the nomination form.
Nomination and Selection Process
  • Nominations must be made by a letter of recommendation and should include a brief summary of the entity or person’s accomplishments and why they deserve this award for their distinguished and dedicated efforts furthering the specialty of family medicine.
  • Explaining why you feel this person deserves the Distinguished Service Award. Include a description of what characteristics make this person/entity stand out, using the judging criteria listed above as guidance.
  • If an individual, provide a current Curriculum Vitae (four-page maximum)
  • A minimum of 3 and a maximum of 5 letters of support (no more than 10 pages total may be submitted)
  • If an individual, provide a recent head-and-shoulders photo of the nominee
  • If an entity, provide a logo
  • Nominations may be received from members or MAFP staff
Judging Criteria
  • The MAFP Member Services Commission will select finalists from all nominations. 
  • The MAFP Member Services Commission shall review all finalists’ materials and select the top candidate to present to the MAFP Board of Directors
MAFP reserves the right to use discretion whether or not to award a nominee annually.
