Program Information
The ISAH Project is a 12-month Case Management program for parents and caregivers with children ages 0-18 years. Eligibility is based on several factors including participation in certain programs, income, and program specific goals.
To begin the application process, please complete the following information. Once the information is received, as staff member will contact you via your preferred method of contact to provide additional information on the application process.
Please note that the application process is not completed until all requested information and forms are received by MCCAA staff. Once all information needed to determine project eligibility is received and processed, you will be notified of your eligibility status.
