* 4. I (Parent Name) give my child permission to participate in the proposed activities with the UOG TRIO Educational Talent Search (ETS) Workshop #2 on November 16, 2019 (Saturday). I understand that my child will be participating in ETS activities from 9:00 AM to 3:00 PM. I understand that UOG TRIO ETS will take all reasonable precautions to prevent accidents, injuries, loss of life and property affecting my child. However, I also understand that as, an adolescent, (my/our) child should also take the responsibility to avoid dangerous and risky situations. In keeping with these understandings, I release the University of Guam, UOG employees, TRIO Programs, UOG ETS program and staff, and any staff and volunteers from any and all liability resulting from any accidents, injuries, loss of life, cost or loss of property, which might occur. THIS FORM IS DUE ON NOVEMBER 13, 2019 (WEDNESDAY) BY 5:00 PM.

I also understand that ETS will not provide transportation, and that parent(s)/legal guardian(s) are responsible for transportation to and from this activity. Students should be dropped off no earlier than 8:30 AM and picked up promptly at 3:00 PM, as UOG and ETS staff will not be available before 8:30 AM and after 3:00 PM to supervise students.
