Information Consent

I am a Clinical Associate Professor in the School for the Future of Innovation in Society and Consortium for Science, Policy and Outcomes (CSPO) at Arizona State University (ASU). We are conducting a research study to collect public views about driverless mobility. You have been selected to participate in a public forum taking place on Saturday, August 3rd, at 9:30am at the University of Buffalo.

As indicated in the consent form you completed during the application process, you will be spending 8 hours participating in group discussions. We will be collecting your anonymous pre and post event surveys, answer sheets, taking notes of our conversations, and audiotaping but not recording individual names.

Your participation in this study is voluntary.  Participants will receive a $100 stipend after the completion of the forum. You must be 18 or older to participate in the research study.

Participants in the study will have the opportunity to learn about and express their views on driverless mobility. There are no foreseeable risks or discomforts to your participation.

If you have any questions concerning the research study, please contact the research team at: mahmud.farooque@asu.edu or 202-446-0397. If you have any questions about your rights as a subject/participant in this research, or if you feel you have been placed at risk, you can contact the Chair of the Human Subjects Institutional Review Board, through the ASU Office of Research Integrity and Assurance at (480) 965-6788.

Thank you for your participation in Our Driverless Futures.

Mahmud Farooque, Ph.D., Clinical Associate Professor, Arizona State University
