Dear participant,
My name is Brittany Hoang and I am a graduate student in the Family Nurse Practitioner Program at San Jose State University. For my final project, I am studying the issue of lack of access to specialized ostomy care and education for ostomates. As you may already know, ostomates are often forced to discharge from the hospital after surgery without the techniques to care for their potential post-surgical complications in addition to mental and physical changes that come with having a new stoma. In my own practice as a Certified Wound and Ostomy Nurse, I have noticed the challenges many patients that I see in my ostomy clinic face in relation to accessing the education and care they need once they are home and expected to manage their ostomy care on their own.
Because you are a person with an ostomy, I am inviting you to participate in this research study by completing this survey.
The following questionnaire will require approximately 5 minutes of your time to complete. There is no compensation for responding nor is there any known risk. In order to ensure that all information will remain confidential, please do not include your name. Copies of the completed surveys will be seen by me and my project advisor. If you choose to participate in this project, please answer all questions to best of your knowledge. Participation is strictly voluntary and you may refuse to participate at any time.
