The DOT(WA) AGM is being held on Tuesday the 23rd of August 2022. If you were a 2021-22 member of DOT(WA) and are unable to attend the AGM, please consider completing this form to nominate a proxy to vote on your behalf at the meeting. The proxy MUST be in attendance at the meeting AND they must have been a 2021-22 member of DOT(WA).
