Thank you for participating in this survey. Your feedback is important and will help to contribute our understanding of social anxiety and future thinking. Please read the following information before beginning the survey.

PROJECT TITLE: Social anxiety and future thinking

Dear Participant,

You are invited to participate in our research study on social anxiety and future thinking patterns. This study gives you the opportunity contribute to our understanding of the different characteristics of social anxiety; in particular how levels of social anxiety affects thinking about future social events. This study is part of a Graduate Diploma in Psychology thesis and is being conducted by Ms. Alice Hativagone under the supervision of Dr Beyon Miloyan from the School of Health Sciences and Psychology at the Federation University Australia.

Participants between the ages of 18 and 65 are invited to participate in an online questionnaire which will take approximately 15- 20 minutes to complete. The questionnaire will ask for background information (age, gender, education level and employment status), feelings over the past two weeks, as well as questions relating to social anxiety and future thoughts concerning social situations. Please free to pass it on to others that you think will be interested.

Participation in this study is voluntary. You have the right to discontinue at any time without explanation or penalty. Questionnaire responses will be completely anonymous and unidentifiable. Therefore, it will not be possible to withdraw your data once your survey has been submitted.  

The anonymous data that is gathered for the purpose of this study will only be available to the named researchers and will be stored for a period of 5 years, in a secure location at the University. Any reports originating from this data will contain only group information and therefore remain confidential.

Participation in this study should involve no physical or mental discomfort. In the unlikely event of distress due to participation in the study, you are free to withdraw your participation and are encouraged  to contact Lifeline (13 11 14) Beyond Blue (1300 224 636) or your University’s Student Counselling Service. If you are a Federation University student, the number for the Counselling Service at Mount Helen Campus is (03) 5327 9470 and at Gippsland Campus is (03) 5122 6425.
