As part of the implementation of shared care for women with early breast cancer at Mater Hospital Brisbane, we are assessing the acceptability of the shared follow-up and survivorship model of care to GPs through a voluntary online questionnaire. The survey comprises 10 questions and will take no more than 10 minutes to complete. All data collected is non-identifiable. Consent will be implied by completion of the online questionnaire. Withdrawal of your consent once the survey is completed is not possible as all data collected is non-identifiable. Results of the study will be distributed via mail to all GPs listed as participating in shared care. This project has been approved by the Mater Misericordiae Ltd Human Research Ethics Committee HREC/MML/70707 (V2).
