Idaho Academy of Nutrition and Dietetics Awards Submission Survey

Purpose of this survey

The Board encourages all members to participate in our Affiliate Member Awards: Recognized Young Dietitian of the Year, Emerging Dietetic Leader, Outstanding Dietitian of the Year, Recognized Nutrition and Dietetics Technician of the Year, and Lifetime Achievement Award.

Any member can nominate a colleague. Nominations may also be submitted from Academy members from other states. If you are not an Idaho Academy member, please include your name and member number with your submission.

We also encourage self-nomination.

Nominations may be made at any time during the year but must be received by March 1st of the award cycle.

Collect the required information before submission.

Review survey questions to determine what information is needed for the various awards.

University Programs are encouraged to submit the following two awards: Outstanding Dietetic Student Award and Preceptor Award.

Other awards include the Gem Award and Service Awards.

Awards are presented at the Annual Meeting each spring.

Lifetime Achievement Award Complete questions 2 through 6. This award requires three letters of support. Attach a photo in question 7.

Outstanding Dietetic Student Award (must be from an ACEND Program) Complete questions 2, 8 (bio) and 9 (photo).

Preceptor Awards (must be from an ACEND Program) Complete questions 2, 8 (bio) and 9 (photo).

Gem Award Complete questions 2 and 4; if applicable, include a photo in question 7.

Special Award (service or other significant contribution) Complete questions 2 and 4; include a photo in question 7 when applicable.
