Instructions:
We are collecting information about the treatment of Chronic Disease in Dubbo and Coonamble on behalf of the Western NSW Primary Health Network.
This survey will take just a few minutes of your time and will help us to compare the services on the ground with the need, to assist the PHN with their future Chronic Disease service planning.
In the first part of the survey we ask the name of your Organisation name and a couple of very brief ‘yes’ and ‘no’ questions. It is really important that you complete this section.
The second part of the survey is only for those services that specialise in treating specific chronic diseases. For example, Diabetes Educators.
