We are conducting a very important evaluation of our school. Thank you for taking the time to reflect on the following questions and answer them honestly. Your responses will help us to improve the De La Salle experience in all areas.
HOW THIS SURVEY WORKS:
1. This survey will take approximately 30 - 45 minutes to complete. If you begin taking this survey and need to stop for any reason, YOU MUST come back to THE SAME COMPUTER OR DEVICE to finish the survey, if you want your previous answers to be saved. This means that if you start the survey in the library, you must return to that SAME COMPUTER in the library to finish the survey if you want your original answer choices to be saved. We recommend that you take this survey on YOUR OWN PERSONAL DEVICE (phone, tablet, or computer). That way you don’t have to finish it in one sitting.
2. YOUR NAME AND STUDENT ID NUMBER MUST BE ACCURATELY TYPED INTO THE SURVEY AND THE SURVEY COMPLETED for you to be cleared to register for classes for the 2019-2020 school year. The reason we are asking for your name is to verify that you have completed a survey. Your answer choices will be stored separately from your name. No teachers, counselors, or parents will ever see your specific answer choices (unless there is the threat of physical harm). You may skip any question that makes you feel uncomfortable.
3. Most of the questions in this survey asks for your "degrees of agreement" with statements about De La Salle. We would like to know how strongly you agree or disagree with a statement that we offer you. PLEASE READ THE QUESTIONS CAREFULLY BEFORE SELECTING YOUR ANSWERS.
4. Please note that after most questions you will see some letters and numbers in parentheses, e.g. [A24b] or [D25bii]. This is merely an internal reference to sections of our report.
