KK Pilot Programme Evaluation Questionnaire
The purpose of this survey is to find out the experience of participating FMS advocates in using the DLP web app in the Klinik Kesihatan setting. Responses collected will be compiled & presented by Dr Sri Wahyu during the Post-Pilot FMS Feedback Session (July 14, 3pm-6pm) for further discussion.
